10.1 For applicant who will pay his own travel expenses:

10.1 A If employee, original employment certificate & ​employment contract;

10.1 B If businessman, business registration certificate

10.1C Original bank certificate (savings or current ​account from commercial or rural banks only) with ​original bank official receipt

*The bank certificate must indicate the following details:

• account name, account number, account opening ​date (must be 6 months active)
  
• type of account
  
• average daily balance for the past 6 months and ​to-date/current balance
  
• with checkwrite.
  

10.1 D Statement of Accounts detailing transactions ​for 6 mos prior to applying for visa ( original ) or ​Passbook detailing transactions for 6 mos prior to ​applying visa (photocopy all pages).

10.1 E Latest ITR with BIR received stamp

10.2 For applicant whose trip to be sponsored by their

parents/children/spouse:

• For student, photocopy of student ID.
• PSA copy for proof of relationship such as birth ​certificate, marriage contract of all concerned parties, ​linking the applicant to the sponsor
  
• If employee, sponsor’s original employment certificate ​& employment contract; if businessman, sponsor’s ​business registration certificate
  
• Sponsor’s original bank certificate (savings or current ​account from commercial or rural banks only) with ​original bank official receipt
